Natural Vet for Pets requires this form, which includes the Medical History and Acceptance Letter to be filled out before making an appointment with Dr. Hershman.
Dr. Hershman needs to learn about your pet before any consultation and to receive your acceptance of homeopathic treatment. The form is divided into sections and should be reviewed and answered as best possible. Your insight and observations will provide Dr. Hershman with a more thorough picture of your pet's health.
